Providers are paid for three separate outcomes: 1) Discovery profile; 2)Securing a job; and 3) Retention of job.The purchase, modification or repair of assistive technology for increased functionalinvolvement of individuals in their activities of daily living. This service also includesindependent living technology which includes remote monitoring. In general, there is alifetime limit of 10,000 with exceptions and a variance process to request technologyabove the limit.This service is available for individuals age 21 and older and includes comprehensiveassessment, the development of and revisions to a behavioral support plan thatincludes strategies to support the individual based upon assessment, and the provisionof interventions and training to individuals, staff and caregivers. There are two levels ofservice that reflect the differing levels of provider qualifications and the needs of theindividual.This service informs an individual about competitive integrated employment and howand whether it will result in increased economic self-sufficiency and/or net financialbenefit through the use of various work incentives. It will also provide information onessential benefit programs.Chore services consist of services needed to maintain the home in a clean, sanitary, andsafe condition. Chore services consist of heavy household activities such as washingfloors, windows, and walls; tacking down loose rugs and tiles; moving heavy items offurniture in order to provide safe access and egress; ice, snow, and/or leaf removal; andyard maintenance. It is provided to individuals living in privatehomes for the purposes of providing supervision or assistance that is designed toensure the individual’s health, safety and welfare or to perform activities of daily livingfor the individual.This service assists unpaid caregivers and/or paid support staff in carrying out anindividual’s treatment/service plan with the goal of improving or sustaining theindividual’s health status and improve his or her independence and inclusion in thecommunity. The service may include assessment, plan development, training andmonitoring.This service is limited to payment for the following: tuition, general fees, on campuspeer support, classes to teach individuals who are deaf and adult education or tutoringprogram for reading or math instruction. Individuals must have an employmentoutcome or an outcome related to skill attainment or development which isdocumented in the service plan and is related to the Education Support need. There isa limit of 35,000 lifetime limit toward tuition and a 5000 limit per semester of oncampus peer support.This service provides professional training and counseling services for unpaid familymembers or caregivers who provide support to an individual. It is intended to develop,strengthen and maintain healthy, stable relationships among the individual and allmembers of the individual’s informal network, to support achievement of the goals inthe individual’s service plan.Other ServiceOther ServiceODP Service . Through the provision of this service, individuals willacquire, maintain, or improve skills necessary to live in the community, to live moreindependently, and to participate meaningfully in community life. Services consist ofassistance, support and guidance in the general areas of everyday living. In theCommunity Living Waiver, Life Sharing is provided for participants in Needs Group 1 or2 or those who need less than 30 hours per week of this service.ConsolidatedComm. The initial session of Music Therapy, Art Therapy orEquine Assisted Therapy must include an assessment of the individual's need for theservice. Code Chapter 6310 are characterized as transitional residential programs incommunity settings for individuals with chronic psychiatric disabilities. This service isfull-care CRRS for adults with intellectual disability and mental illness. Full-care CRRS foradults is a program that provides living accommodations for individuals who arepsychiatrically disabled and display severe community adjustment problems. A fullrange of personal assistance and psychological rehabilitation is provided for individualsin a structured living environment.A licensed Community Home is a home where services are provided to individuals withan intellectual disability or autism. A community home is defined in regulations (55 Pa.Code Chapter 6400) as, "A building or separate dwelling unit in which residential care isprovided to one or more individuals with an intellectual disability or autism". Code §6400.3(f)(7) licensing regulationsexclude community homes that serve 3 or fewer individuals with an intellectualdisability or autism 18 years of age or older who need a yearly average of 30 hours orless of direct staff contact per week per home.These are services that are provided to supervise and support individuals living inprivate homes on a short-term basis for planned or emergency situations, giving theperson(s) normally providing care a period of relief that may be scheduled or due to anemergency. Competitive integrated employment refers to full or part-time work atminimum wage or higher, with wages and benefits similar to workers withoutdisabilities performing the same work, and fully integrated with coworkers withoutdisabilities.Individuals receiving this agency managed service will be supported to live in a privatehome owned, leased or rented by the individual. This service will support individuals toacquire, maintain or improve skills necessary to live more independently and be moreproductive and participatory in community life. In the Community Living Waiver,Supported Living is provided for participants in Needs Group 1 or 2.This service is available to individuals who elect to self-direct their services througheither the Agency with Choice or Vendor Fiscal/Employer Agent models.
